Preparing for a diabetes follow-up

Use your visit to review patterns, goals, and ongoing care needs.

Bring the information you already have

If home monitoring is part of your existing plan, bring your recorded readings and the circumstances around them. Note changes in eating, activity, symptoms, or your ability to follow the care plan. Do not start a new testing schedule based only on this guide.

Understand what A1C can show

An A1C test reflects average blood glucose over about three months. Your individual goal and testing schedule depend on your health and diabetes history. Ask what your result means in context rather than comparing only with another person’s target.

Set the agenda for follow-up

Ask which preventive checks and outside appointments are due, how relevant results will be reviewed, and when to return. Tell the clinician about practical barriers such as the cost of testing, transportation, or difficulty keeping appointments.

Keep an urgent-care plan

Ask your care team which symptoms or readings require urgent action and keep that individualized plan available. Severe confusion, fainting, breathing difficulty, or inability to stay awake is an emergency. Do not wait for a routine appointment or website reply.

Emergency symptoms?

Call 911 or go to the nearest emergency department. Do not wait for a routine appointment, phone call back or email reply.
