Preventive wellness

Discuss age-appropriate prevention and wellness. Recommendations depend on your history, risk factors, prior testing and clinical assessment.

• Wellness and preventive care visits
• Screening recommendations based on individual needs
• Routine labs when clinically appropriate
• Practical, achievable health goals

Chronic condition follow-up

Ongoing conditions benefit from a clear follow-up plan. Visits may include reviewing symptoms, measurements, prior results and how the current plan is working.

• High blood pressure follow-up
• Diabetes and other chronic condition monitoring
• Review of relevant labs and progress
• Discussion of when further evaluation is needed
