Medication Refill Preparation Checklist
Prepare for an online medication refill evaluation with the medication name, exact dose, pharmacy, allergies, prior prescriber information, recent readings, and relevant laboratory results.

Have these ready before the refill evaluation
- Medication name (generic name if known)
- Strength and exact dose
- How often you take it
- How long you have taken it
- Reason it was prescribed
- Original bottle/packaging or a clear label photo
- Preferred pharmacy name and location
- Medication and food allergies
- Other prescriptions, over-the-counter medicines, vitamins, and supplements
- Recent side effects, missed doses, or medication changes
- Recent relevant blood pressure, glucose, or lab results if available
- Regular prescriber/PCP contact information if available
